Disclaimer

I understand that mailing imaging studies for review prior to becoming a patient is a courtesy offered by Princeton Neurological Surgery for the sole purpose of answering basic questions concerning brain and spine conditions in general and to assist in learning whether the type of condition or lesion shown is similar to those that have been successfully treated at Princeton Neurological Surgery. Unless and until an individual actually becomes a patient of the practice and is examined by Dr. Lipani, the practice cannot provide any advice concerning a patient’s particular medical condition or treatment.

Although every effort will be made to protect the mailed imaging material, I understand that mail correspondence is subject to loss, theft, mishandling, and damage. I realize that the patient’s name and birth date may appear on mailed material. I also understand that I have the option to upload the imaging studies on the Princeton Neurological Surgery website for review. I do not hold Princeton Neurological Surgery or Dr. Lipani responsible or liable for lost, damaged, or unintended use of mailed imaging materials.

I have read and accept the terms of this disclaimer.
